长江中下游流域是我国经济发展最快的区域之一,处于经济转型期,沿江居民的生活方式、饮食结构发生了巨大的变化,加之城镇化带来的空气、水体污染等环境危险因素增加,该地区居民的健康水平和环境危险因素需要系统评估。本课题基于泰州与常州队列,进一步优化江苏省自然人群队列的建设,完成江苏省沿江地区6万自然人群队列建设。建立人群队列动态随访系统和高效的终点事件发生追踪系统,对人群进行持续随访;构建常见慢性病疾病风险模型,为地方政府制定预防政策提供科学依据。本课题还将建成华东区域自然人群队列数据管理与共享服务平台,整合项目收集的10万人群数据资源及生物样本信息,为本项目及整个精准医学研究提供队列资源。目前,已完成华东区域自然人群队列项目相关技术与规范建设,建成6万标准化的江苏省沿长江地区自然人群队列,完成数据库与生物样本库建设,简要分析了队列人群特征及自报告发病情况;建成适于区域队列的实时、高效的随访方案,并展开了随访工作;建成华东区域队列数据平台;完成常见慢性病风险评估和预测模型构建。The middle and lower reaches of the Yangtze River basin is one of the fastest growing regional economy in China with a period of economic transition.Great changes have taken place in the way of life style and diet structure of residents along the river.Risk factors of environment,such as air and water pollution,increases under the circumstance of urbanization.The health level and environmental risk factors in the area need to be systematically assessed.Based on the Changzhou Cohort and Taizhou Cohort,this study further optimizes the construction of the natural population cohort in Jiangsu province,and completes the construction of the natural population cohort of 60,000 people along the riverside region of Jiangsu province.A dynamic follow-up system and an efficient end-point event tracking system will be established to continuously follow up the population.The risk prediction model of common chronic diseases is constructed to provide scientific basis for local governments to formulate prevention policies.This study will also build a data management and sharing service platform for natural population cohort in east China,integrate data resources and biological sample information of 100,000 people,provide cohort resources for precision medical research.At present,the study has completed the construction of related technologies and specifications of the natural population cohort in east China,completed the establishment of standardized natural population cohort of 60,000 people along the Yangtze River in Jiangsu province,completed the construction of the database and biological sample database,and briefly analyzed the characteristics of the population and self-reported diseases.A real-time and efficient follow-up plan suitable for the regional cohort was built,and the follow-up work was carried out.The cohort data platform in east China was initially built.The risk assessment and prediction models for common chronic diseases were completely constructed.

华东地区地理环境多样,社会经济发展水平不一,人群遗传结构复杂,本项目充分考虑精准医学需求及华东地区特点,基于各地既有前瞻性人群队列,参考国家示范队列建设要求,制定了华东区域自然人群队列建设相关规范、标准化问卷及调查指南、队列人群基线及随访调查的操作流程和技术规范等。截止中期,基本完成了任务书中的10万自然人群队列的建设目标,相应的生物标本库也已完成入库。依托基线数据库,初步描述了华东地区队列人群的人口学和行为生活方式特征,分析了其主要慢性病的流行现况。各个课题组对卫生、公安、社保、民政、医保等不同来源的资料进行了整合,建立了高效的信息化随访数据系统,可以实时获取队列人群的发病及预后和死亡,以及暴露、迁移和失访等信息,及时发现常见慢性病的发病和死亡等结局事件。与此同时,课题组均启动了队列对象重复基线调查以及面对面随访工作,并完成了11000余人重复基线调查和15000余人面对面问卷随访。项目已经完成了队列数据融合、数据质控与资料统一共享应用等相关技术的研发工作,形成了一套支撑至少10万人群数据融合的技术体系,基本建成华东地区自然人群队列数据共享平台。项目组成员发表了科研论文26篇,所有文章均标注本课题或项目资助;依托本项目,项目组还出版专著4部,软件著作权4件,申请专利2项。项目建设期间,还支撑国家级科研项目2项,申请成立山东省胃癌健康医疗大数据科技创新联盟。 This project fully considered the requirements of Precise Medicine and the characteristics of Eastern China,established the standardized questionnaires and investigation guidelines,as well as the operation procedure and technical specification for the cohort baseline and follow-up investigation in order to build the natural population cohort in Eastern China.And it was based on the existing prospective cohorts in different regions and referred to the requirements for construction of the national demonstration cohort.By the mid-term,the building goal in the task document-that is to build a 100,000 population cohort-has been essentially completed,and the corresponding biological samples have been put in the biobank.Using the baseline database,geographic and lifestyle characteristics of the cohort population in Eastern China has been preliminary illustrated,including analyzing its prevalence of the major non-communicable diseases.Each research team integrated information from different resources such as health department,public security bureau,social insurance department,civil affairs,and health insurance department,etc.The highly effective information follow-up data system is well established,which could be useful when acquiring information about cohort population’s disease incidence,prognosis and death,exposure,migration,and loss to follow-up,etc.,and also be available when identifying end-point events such as morbidity and death for the common non-communicable diseases in time.All of the research team initiated the repeated baseline investigation as well as the face-to-face follow-up for the cohort participants,completed the task of about 11,000 people and 15,000 people respectively.This project has already completed the scientific work of the related technology such as the cohort data fusion,data quality control,as well as the information united to share and use.It has formed a technical system that could support at least 100,000 cohort population’s data fusion,essentially established a data-sharing platform of the natural population cohort in Eastern China.Members in the project team published 26scientific papers.Relying on this project,the project team also published four treatises,owned four software copyrights,and applied for two patents.This team supported two national research projects and applied for establishing the health medical big data technology and innovation alliance of gastric cancer in Shandong.
