项目标书摘要:心脏衰竭是全球性的健康问题。慢性心脏衰竭病患的有效治疗必须依赖正确评估其导致心脏衰竭的病态生理学、密切监视病情以及给予个别调整的药物。虽然目前药物与器械治疗的进步,部分心脏衰竭病人仍然面临疾病的恶化、反覆的住院与早期死亡。心脏衰竭恶化的其中之一原因就是一氧化氮的生物利用率下降以及血管内皮功能受损。而血管内皮功能与血液中内皮前躯细胞的关系已经被证实,反映出血管内皮受损後会经由内皮前躯细胞来加速内皮修补。对心脏衰竭而言,血液中CD34阳性细胞的数目於疾病早期是增加的,相对的,於严重心脏衰竭时是减少的。此外,内皮前躯细胞更可以预测慢性心脏衰竭病人的死亡。研究显示相关於心脏衰竭的病理机转诸如内皮功能不良,神经性荷尔蒙的活化,发炎以及氧化压力,也都会调控内皮前躯细胞的活动与功能。然而,於急性心脏衰竭的病人,我们尚不了解内皮前躯细胞的作用。因此,本研究的目的在於探讨内皮前躯细胞在急性心脏衰竭而住院的病患的角色及其可能的治疗效果。

Application Abstract: Heart failure is a major health problem worldwide.The effectiveness of chronic heart failure therapy depends upon an accurate assessment of the underlying pathophysiology,accompanied by close patient monitoring and tailored treatment.Despite the proven benefits of current pharmacological treatment and device therapy,patients with heart failure still experience progressive symptoms,repeated hospitalizations,and premature death.One of the pathogenesis of worsening heart failure is the decreased bioavailability of nitric oxide and impaired endothelial function.A significant relation between endothelial function and the number of endothelial progenitor cells(EPCs)in healthy volunteers is noted,which reflects an increased endothelial cell turnover by EPCs after endothelial injury.As regards heart failure,CD34+cells count increases in the early phase of the disease,whereas their number is reduced in patients with severely symptomatic heart failure.Furthermore,EPCs could also predict all-caused mortality in chronic heart failure.Accumulating data suggests that all mechanisms which are implicated in the pathogenesis of heart failure including endothelial dysfunction,neurohumoral activation,inflammation,and increased oxidative stress may have distinct role in the mobilization and function of EPCs.However,the impacts of circulating EPCs on acute decompensated heart failure have not yet to be determined.We therefore investigate the exact role of EPCs in acute heart failure and their potential therapeutic implications.
