“健康中国”战略倡导以价值为核心的医疗支付机制促进医患协作、全面提升医疗服务质量。本文关注患者兼具“患者”和“医生助理”双重角色的慢性病管理过程,基于健康价值的内在动态性探讨固定医疗支付、基于服务的医疗支付和基于价值的医疗支付三种机制对自我健康管理决策和医患协作的影响。研究发现:基于价值的医疗支付虽然可以引导医患协作,提升健康价值,但是会抑制患者自我健康管理的努力水平;固定医疗支付无法有效激励医疗服务提供方努力,导致慢性病管理系统效用最低;基于服务的医疗支付能协调医患协作,实现最优的健康价值和系统效用。因此,在消费者既是服务的“支付方”又是服务的“协作方”、且服务价值具有时变特征的情境下,应当慎重实施基于价值的支付机制。

在人类社会发展进程中,健康问题始终不容忽视。由此提出一个具有普适性的数字健康平台,不仅可以提供线上线下双线医疗服务,而且汇集了中医、西医、蒙医等多重类别的医学人才和技术,提供多样性的远程医疗服务。本研究应用溢出效应理论对数字健康平台进行建模,分别分析了不考虑溢出效应和考虑溢出效应两种策略下平台的定价决策和预期收益。结果显示溢出效应的大小对平台预期收益的影响较大,并且通常情况下平台考虑溢出效应时的预期收益将高于不考虑溢出效应时的预期收益,但当溢出效应极低时不考虑溢出效应时的预期收益将高于考虑溢出效应时的预期收益。

PurposeThe study aims to address a critical gap in existing healthcare payment schemes and care service pricing by recognizing the influential role of patients' decisions on self-management efforts. These decisions not only impact health outcomes but also shape the demand for care, subsequently influencing care costs. Despite the significance of this interplay, current payment schemes often overlook these dynamics. The research focuses on investigating the implications of a novel behavior-based payment scheme, designed to align incentives and establish a direct connection between patients' decisions and care costs. The primary objective is to comprehensively understand whether and how this innovative payment scheme structure influences key stakeholders, including patients, care providers, insurers and overall social welfare.Design/methodology/approachIn this paper, we propose a game-theoretical model to incorporate the performance of self-management with the demand for healthcare service, compare the patient's effort decision for self-management and provider's price decision for healthcare service under a behavior-based scheme with that under two implemented widely payment schemes, that is, co-payment scheme and co-insurance scheme.FindingsOur findings confirm that the behavior-based scheme incentives patient self-management more than current schemes while reducing their possibility of seeking healthcare service, which indirectly induces the provider to lower the price of the service. The stakeholders' utility under various payment schemes is sensitive to the cost of treatment and the perceived health utility of patients. Especially, patient health awareness is not always benefited provider profit, as it motivates patient self-management while diminishing the demand for care.Originality/valueWe provide a novel framework for characterizing behavior-based payment schemes. Our results confirm the need for modification of the current payment scheme to incentivize patient self-management.

To promote the application and development of healthcare data, China has conducted a pilot project of the National Healthcare Data Center. By treating the pilot project as a quasi-natural experiment and employing a staggered DID model, this paper provides causal evidence that this pilot project for digital health infrastructure has a positive effect on the innovation of pharmaceutical and healthcare companies, which is mainly reflected in an increase in applications and citations for invention patents. The effect can also be correlated with the increase in R&D intensity, R&D efficiency and collaborative R&D. The mechanism analysis shows that from the perspective of resource utilization, digital health infrastructures affect corporate innovation in at least three ways, i.e. alleviating the resource misallocation, increasing the resource slack and using external resources.

The Salop model was used to simulate competition after investment and development in internet hospitals (IHs) were made due to patients’ selection in the medical service market. The optimal investment level and value of IHs were then derived from the total welfare decision. Results of the numerical simulation demonstrated that the investment value of the IH in the online/offline direct-to-patient mode required multi-stage and multi-investment optimization decisions and can be increased by improving patients’ cognition and acceptance. Strategic insight to achieve the balance between cost payments and dynamic value is needed to decide the investment value of the IH, especially in the early stage. From the perspective of welfare, the IH should serve more patients in remote areas. The capability and maturity of creating value on investment (VOI) need to improve with the applications proportionally increasing. Furthermore, investment by a public-private partnership will mean that the IH will start up with less capital, an inclusive strategy, and create higher welfare. © 2023 Southeast University. Consumption compensation is the driving force for motivating each supply chain link to set up information nodes, which is of great significance for promoting informatization, modernization, transformation, and upgrading of the supply chain system. This study simulates the evolution process of strategic selection for each supply chain link to set information nodes independently or collectively under the unconstrained government rewards conditions and punishments. The main conclusions are as follows: (1) When each link in the supply chain provides source tracing information independently, the vendor chooses to set up information nodes for the links with large information cost compensation coefficients. (2) When the vendor provides source tracing information cooperatively if the collaborative benefit is less than the cooperation cost, the information node will be set to the link with the large compensation coefficient. If not, information nodes will be set in each link. (3) Under the government incentive mechanism, enterprises are willing to cooperate. The punishment mechanism helps avoid situations where neither side sets up information nodes. The compensation coefficient is a prerequisite for mechanisms to work.

Background: The recent surge in clinical and nonclinical health-related data has been accompanied by a concomitant increase in personal health data (PHD) research across multiple disciplines such as medicine, computer science, and management. There is now a need to synthesize the dynamic knowledge of PHD in various disciplines to spot potential research hotspots. Objective: The aim of this study was to reveal the knowledge evolutionary trends in PHD and detect potential research hotspots using bibliometric analysis. Methods: We collected 8281 articles published between 2009 and 2018 from the Web of Science database. The knowledge evolution analysis (KEA) framework was used to analyze the evolution of PHD research. The KEA framework is a bibliometric approach that is based on 3 knowledge networks: reference co-citation, keyword co-occurrence, and discipline co-occurrence. Results: The findings show that the focus of PHD research has evolved from medicine centric to technology centric to human centric since 2009. The most active PHD knowledge cluster is developing knowledge resources and allocating scarce resources. The field of computer science, especially the topic of artificial intelligence (AI), has been the focal point of recent empirical studies on PHD. Topics related to psychology and human factors (eg, attitude, satisfaction, education) are also receiving more attention. Conclusions: Our analysis shows that PHD research has the potential to provide value-based health care in the future. All stakeholders should be educated about AI technology to promote value generation through PHD. Moreover, technology developers and health care institutions should consider human factors to facilitate the effective adoption of PHD-related technology. These findings indicate opportunities for interdisciplinary cooperation in several PHD research areas: (1) AI applications for PHD; (2) regulatory issues and governance of PHD; (3) education of all stakeholders about AI technology; and (4) value-based health care including "allocative value," "technology value," and "personalized value."

First, the evolutionary game model of patients and healthcare providers participating in information sharing under four value-based healthcare delivery modes, i.e., traditional medical model, collaborative care, self-managed care(SMC), and healthcare value co-creation(HVC), is constructed. Second, co-creators' initial participation ratio, information quality, which can trigger deviation from equilibrium sustainability in the dynamic process, is analyzed. Results show that extensive patient participation is more helpful to sustainable platform operation than provider participation.
